Online Registration for the Symposium is available, including full, single-day, and mulit-day registrations. If you are an invited speaker, category day speaker, an approved concurrent session presenter or an approved paper presenter please see information below regarding discounted rates.
Payment for online registrations are only being accepted in the form of Visa, MasterCard, and American Express. Your credit card will be charged as "Commissioned Officers Foundation" or "COF". Our tax ID# is 52-2258463
The Symposium registration fee includes free admission to participate in the Surgeon General's 5K Run/Walk. Sign-up for the Run/Walk.
Cancellation Policy: Refund requests must be received IN WRITING by May 5, 2008. Email to info@phscofevents.org
All rates include a $35 non-refundable processing fee. Returned checks incur a $35 fee.
Student/Junior Officer Scholarships
If you are interested in a Student or Junior Officer Scholarship please review the guidelines in the right-hand column. You may apply for a Student Scholarship by registering online at the Student rate. For Junior Officer Scholarships you must submit the form in the right-hand column. All applications will be reviewed and recipients notified within one week of applying. Individuals who submit a paid registration will be ineligible for a scholarship.
The Junior Officer Scholarships are funded and approved by the PHS Commissioned Officers Foundation. The grants are not connected to, reviewed by or approved by the Junior Officers Advisory Group. Questions may be directed to info@phscofevents.org
Speaker Registration
If you are an invited speaker, category day speaker, an approved concurrent session presenter, or an approved paper presenter, please register at the discounted speaker rate online.
PLEASE NOTE: The database will be checked weekly to verify that those registered at the speaker rate are speaking at this event. If you register at the speaker rate and are not a speaker we will automatically change your registration fee to the Non-Member rate. No refunds will be granted.
